Question on obsessive thoughts

For those of you who experience obsessive thoughts, are they usually pretty much unfounded/unlikey, or based on truth but completely exagerated, or an actual truth?

they are never the truth and they never happen. ever. that being said, I still struggle with them.

if you say, "thoughts, only thoughts", that helps them go away much quicker.
