Session One Question

I just listened to the beginning of this audio
cd and the man says to fill out the evaluation
form at the end of Session 1 and 15 and return it
back?- I couldn't find them? Or is this for the
Online or Coaching sessions perhaps?

Hi Deb,
I listened to the first audio session last night myself. I think what he is referring to is the Anxiety Evaluation Survey in the Guidebook for sessions 1-4 on page 1-7. Hope that helps!

Ah yes that's it. So do we need to rip it out of the book and mail it?
